• Employee/Volunteer Information & Disclosure Statement - all  new coaches must read, sign and return to Coordinator.

• PA Child Abuse (DPW) History Clearance Form - all new coaches need to fill this out and return to Coordinator.

  • Registration Packet Cover Page - checklist used by us for all players

  • 2011 CFA Registration Form - CFA form submitted to CFA - A physician CAN sign this document in "Physician's Affidavit" section, as proof of physical (we also accept the doctor's own physical exam forms as proof.)

  • Parent Participation Form - indication specifics around volunteering

  • Medical Information Card - kept at the field at all times in case of emergency.

  • Birth Certificate - a copy, please, we file and submit this to the CFA as proof of age.
